Please fill out the questionnaire BELOW as soon as you have booked your first facial appointment. This will ensure that the most accurate skin history is taken for consultation or skin analysis.

 

If you are coming in for a wax, the hair must be the size of a grain of rice or longer.  This ensures the hair is long enough to wax. You may exfoliate the area to be waxed the day before your scheduled appointment. If you are on your cycle, you may use a tampon for the duration of the appointment. 

The suite is located inside  "Bella Salon."  There is available parking and is handicap accessible through the street facing entrance. Please check in with front desk when you arrive. 

 

Please arrive 10 minutes early to your first appointment.
